Thank you for your interest in the Ohio Society of Anesthesiologists. To apply for ACTIVE membership you may simply complete the on-line application by clicking on the link below. ASA and OSA share the same application. Once your application is complete it will automatically be sent to the OSA Secretary/Treasurer for approval. You will then be notified by OSA and ASA of your membership. The Annual Dues for OSA Active membership is $250.
